INTRODUZIONE
Possiamo intendere l’evoluzione come una continua e costante assimilazione d’informazioni dal mondo circostante? Del resto, qualsiasi organismo vivente deve poter trarre input dall’esterno per sopravvivere. Più l’organismo è complesso, più dovrà avere dati necessari all’adattamento evolutivo e quindi maggiore deve essere la sua capacità di processare le informazioni necessarie.
Tra tutti gli organismi viventi, l’uomo è sicuramente tra i più complessi, capace di tradurre i suoi pensieri attraverso il linguaggio, acquisendo così una sua unicità. In effetti, la percezione si definisce proprio come l’atto di raccogliere ed elaborare le informazioni provenienti dall’ambiente circostante. I processi percettivi traggono informazioni dai nostri organi di senso, l’udito, l’olfatto, il gusto, il tatto e la vista.
L’evoluzione ha adattato le aree del cervello, assegnando differenti risorse alle parti dedicate ai sensi, in base all’efficienza di queste, ai fini della sopravvivenza.
Il sistema visivo e i processi neurali, legati alla percezione visiva, occupano ampie aree del nostro cervello, quasi un quarto dell’intera corteccia cerebrale. Ciò è indice della predominanza del canale visivo nei processi percettivi umani.
L’obiettivo del presente lavoro di tesi è analizzare gli effetti sulle capacità percettive della deprivazione visiva, nei primi mesi di vita, in bambini soggetti a cataratta congenita. Come vedremo più in dettaglio durantel a trattazione, la cataratta congenita non è altro che l’opacizzazione di una parte dell’occhio, che si chiama cristallino. L’opacizzazione, se congenita e binoculare, comporta una visione come attraverso un vetro opalino, quindi senza possibile discriminazione del dettaglio.
Nel mondo industrializzato, dove l’assistenza pediatrica è presente per la quasi totalità dei soggetti, l’asportazione chirurgica dei cristallini opacizzati e il successivo ripristino di una visione nitida, avviene nei primissimi mesi o anni di vita. Purtroppo in alcune parti del mondo questo non avviene; ci sono aree geografiche ove la situazione politico-economica del Paese e le condizioni di vita delle popolazioni, precludono l’accesso all’assistenza sanitaria. In Paesi, come ad esempio l’India, troviamo moltissimi bambini affetti da questa patologia; essendo di fatto preclusa loro la possibilità d’intervento, questi soggetti vanno incontro a una cecità totale o parziale, per una consistente parte della loro vita.
Negli ultimi tempi, si stanno sviluppando dei programmi di aiuto per queste popolazioni, che rendono disponibile l’intervento di cataratta anche in soggetti con età compresa tra i 4 e gli 8 anni, in una fase quindi ove normalmente il sistema visivo è già sviluppato ed efficiente. Una di queste iniziative è il Progetto Prakash. Lo stesso progetto, si sviluppa in India attraverso un’organizzazione molto articolata, con obiettivi umanitari e scientifici. Il Progetto vede la collaborazione di Università Internazionali, medici, psicologi, operatori del settore oculistico, strutture ospedaliere e istituti di ricerca.
L’obiettivo di tale progetto è quello di coniugare l’aiuto sanitario a popolazioni povere, abbinando la ricerca scientifica su pazienti con caratteristiche molto particolari, rarissime nel mondo industrializzato. Il Progetto Prakash, oltre ai fini umanitari, offre ai ricercatori la possibilità di studiare la percezione in persone alle quali è stato precluso lo sviluppo dello stimolo visivo, che in soggetti normovedenti avviene nei primi mesi e anni di vita. Il protocollo prevede di intervenire su bambini con cataratta congenita, ciechi dalla nascita, con età superiore ai quattro anni. L’intervento, seppur tardivo, oltre a donare la vista ai bambini coinvolti nel progetto, offre preziose informazioni circa le caratteristiche della percezione visiva, in soggetti sottoposti a deprivazione della vista dalla nascita.
Il punto focale, che rappresenta anche il tema specifico del presente lavoro di tesi, è che in tali pazienti è possibile studiare quanto l’apprendimento nei primi anni di vita sia importante per sviluppare le funzioni percettive normali. I pazienti, coinvolti nel Progetto Prakash, sono infatti persone che non hanno potuto “apprendere” a percepire nei primi anni di vita. In questi pazienti, nei quali il normale apprendimento percettivo non è potuto avvenire, è stato possibile analizzare i fattori innati della percezione e quelli appresi, studiando le caratteristiche dei processi successivi all’intervento di sostituzione del cristallino. In sintesi, ciò che i pazienti del Progetto Prakash sembrerebbero indicare, sembra più il frutto di fattori innati, che di quelli appresi.
ll presente lavoro sarà strutturato, nella prima parte, con una descrizione anatomo-fisiologica dei processi della visione. In questa sezione si seguirà il percorso dell’informazione visiva, dal momento in cui entra nell’occhio fino all’elaborazione cerebrale, che porta alla percezione. Nell’ambito di questo percorso ci si soffermerà sugli aspetti funzionali, fino ai risultati degli studi condotti sui bambini sottoposti al trattamento chirurgico.
In seguito, saranno trattati i processi della percezione visiva, con particolare riferimento ai processi innati versus i processi appresi.
Tramite una disamina critica della letteratura scientifica, si tenterà di capire se, nel processo percettivo, sia fondamentale una buona funzionalità visiva, tale da sviluppare correttamente le aree cerebrali implicate nella percezione visiva.
Nell’ambito della corposa ed estesa raccolta di dati del Progetto Prakash, nel presente lavoro di rassegna si è cercato di estrapolare gli studi che interessano gli aspetti cognitivi della percezione. Nello specifico, saranno trattati dettagliatamente e separatamente diversi aspetti della percezione visiva: il contrasto, le illusioni ottiche, il riconoscimento facciale, l’acuità visiva, l’analisi visiva e i rapporti percettivi vista – tatto. Si tenterà di capire come il ripristino del segnale visivo influisca sulle prestazioni percettive nel tempo.
Le conclusioni, oltre a una “summa” dei risultati posti in rassegna, non potranno definire con certezza un’univoca prospettiva, poiché ci si trova in un campo dove il cervello e la sua incredibili plasticità riservano sempre delle eccezionali sorprese. Del resto, non è raro vedere teorie consolidate messe in crisi da scoperte inaspettate, anche grazie a iniziative di grande importanza umanitaria e scientifica, quali il Progetto Prakash.
